Mini Lithotripsy for plantar fasciitis & other chronic conditions

There is both a high-energy and low-energy form of Extracorpeal Shock Wave Therapy (ESWT). Our clinic, Atl Pain Institute, has a mini lithotripsy machine that delivers low energy soft shock waves to the bone of the heel. This is an unfocused treatment, which generates more concentrated therapeutic healing energy to the tissue. It can be done under local or general anesthetics, but most can tolerate in office sessions.  This treatment was originally used for breaking up kidney stones. We have used this as our ‘treatment of last resort’ previously with excellent results. The standard of care for the last 30 years has been steroid injections with limited results. Q: Are Steroid injections for Plantar Fasciitis still the standard of care?

A: At this time, we are recommending low intensity shock wave treatment ahead of steroid injections for plantar fasciitis at Atl Pain Institute.  This is for two main reasons.  Although there is some chance of increased pain for a few days after the treatment, there is no systemic affect on the whole body and the treatment is a walk-in walk-out one.  Also, it seems likely that the long acting corticosteroid would be forced out of the tissue if shock wave was used after an injection, thus moving it away from the site of action.

Q: What’s the biggest weakness in this therapy?

A: There are 2: This treatment is not covered by insurance at this time, but in office sessions are very affordable. The other potential downside to treatment is that it can be uncomfortable at the time of delivery and it can increase pain slightly after a session.

When ESWT is used on normal tissue is generally quite pleasant- no pain, but when used on areas of injured tissue or inflammation, there can be some discomfort associated with treatment.

We are currently using the treatment on a variety different condition including diabetic & peripheral neuropathy, chronic pain, whiplash, non healing fractures, ulcers or diabetic wounds, erectile dysfunction & chemo induced peripheral neuropathy.

So far, we have found that patients enjoy the near immediate decrease in pain & restoration of improved range of motion.

Q: What is the cost of Soft Shock wave therapy?

A: The treatment is often done three or four times at an interval of 4 to 7 days apart. The cost of the sessions can range from $100 dollars to $300 per session. In office financing is available if necessary.
